10+ Food Frequency Questionnaire Templates in PDF | MS word
A document that is used by the clients or clinicians to categorize the frequency of consumption of certain foods or food items is known as a food frequency questionnaire. It can be used to gather information on the frequency and portion size of the food and beverage consumption over a given period. This questionnaire enables calculated nutritional intakes to be collected during a given period.

How to Frame Your Questionnaire?
The food frequency questionnaire or an FFQ usually includes questions about a set of food and beverage items. There may be some important questions for each food or beverage item asking about the following:
- You may ask about the frequency of consumption. You can include various options through which the respondent will be able to respond if it is never, frequent or infrequent and many other options.
- You can include questions that may be related to the portion size information which may be framed in terms of portion size for each round of consumption or total portion size. You can even ask the respondents to translate the usual consumption amount to the number of specified units. For instance, a cup of rice. You can even include portion size images to enhance reporting accuracy.
- Other questions may seek information regarding the frequency of consumption at particular times of the year, or the type or brand that people generally consume.
- Your FFQ can also ask about the frequency of intake and dosages of commonly consumed dietary supplements.
Key Benefits of Using a Food Frequency Questionnaire
- It provides information on the consumption of queried foods and beverages over a specific period. It depends on the breadth of items that are queried, data that can be used to assess total dietary intake or particular aspects of diet. Information resulting from the questionnaire may represent either the usual frequency of consumption only or the total amount that is usually consumed.
- With the help of an FFQ, the resulting data can be linked to yield information about the total dietary intake. Linking data to a nutrient composition database will help you in determining the nutrient intake. It can also help in providing information about the consumption of food groups of particular interest, such as the total amount of fruits, vegetables, or any added sugars.
- FFQ is also used to describe a population’s intake in cross-sectional studies, however, it is not recommended because of their large systematic error.
- FFQs also help in examining certain associations between diet and health or other variables. It is commonly used as one of the main dietary assessment instruments in large prospective studies.
- FFQs are considered to be the only practical methods for capturing diet in retrospective case-control studies.
- This questionnaire is also used to evaluate the effectiveness of interventions to change diet.
Limitations of a Food Frequency Questionnaire
Even though an FFQ has many benefits, it has some limitations as well. Some of them are as follows:
- It may contain systematic errors in it and this error can be at least partially reduced through appropriate statistical modeling or by using a less biased dietary assessment method, etc.
- Some detailed information about food preparation, specific food, and other beverages that are consumed along with its brands and information about intake is missing in FFQs.
- Any single FFQ may not reflect the eating patterns of a given population because an FFQ is composed of a pre-specified food list. Therefore, the performance of a particular FFQ in a specific population may not show its performance in a different population.
Strengths and Weaknesses of a Food Frequency Questionnaire
Apart from having its benefits and limitations, food frequency questionnaires have their strengths and weaknesses as well.
Strengths
FFQs are better at estimating the ‘usual diet’ due to a longer recall period than the 24-hour dietary recall or a 24-hour weighed food records. It also helps in capturing individual-level dietary patterns and it can be easier and less time-consuming to implement than a 24-hour dietary recall, in case the food list is relatively short.
Weaknesses:
A food frequency requires a substantial up-front investment to develop and prove the instrument for a given context or a particular country. The usual frequency of intake is generally prone to some measurement errors, specifically with recall periods that are longer than seven days. It can also be quite time-consuming to administer than a standard 24-hour dietary recall if the FFQ is too long and it would make the respondent fatigued.
Conclusion
Food is something that none can ignore, but even though food is what keeps us healthy, it can also lead to our illness in some cases. Therefore, it is always better to take precautions and maintain a diet and what is better than checking your consumption frequency with the help of a food frequency questionnaire.
