Step 5: Get Details About the Person Filling Up the Questionnaire
Knowing the details of the person filling up the questionnaire gives additional insight. That data should be collected as well. However, don’t go about asking for personal details like name, phone number address, etc. Asking for personal details makes the person less inclined to cooperate. He or she might decide not to fill up the questionnaire at all.
