Dear Mr. Knowles,
I, Jane Doe, the undersigned, am a subcontractor providing electrical services for the renovation project located at 1565 Orphan Road. This document serves as a partial waiver of my right to claim a lien for the electrical work provided up to the date of November 23, 2050.
In consideration of the payment received in the amount of $5,000, I hereby acknowledge that this payment covers the following portion of the total contract price:
I further affirm that upon receipt of this payment, I release any and all lien rights or claims for the described electrical work. This waiver applies only to the extent of the payment received and does not waive any rights for future electrical work.
This waiver is effective as of November 23, 2050, and is contingent upon the actual clearance of the payment in Maverick Electrical Services' account.
By signing below, I acknowledge receipt of the specified payment and waive any lien rights for the described electrical work up to the current date.
