Waxing Client Release Of Liability
This Release of Liability ("Release") is made effective as of [Date] between [Your Company Name], located at [Your Company Address], hereinafter referred to as the "Service Provider," and the undersigned client, hereinafter referred to as the "Client."
I. Purpose of Waxing Service
The Client hereby acknowledges and agrees to undergo waxing procedures at the Service Provider's establishment. The purpose of the waxing service is to remove unwanted hair from specific areas of the body.
II. Acknowledgment of Risks
The Client acknowledges that they have been informed of the potential risks associated with waxing procedures. These risks may include, but are not limited to:
Skin irritation, redness, or swelling.

Burns or skin damage due to improper waxing techniques or excessive heat.
Infections or ingrown hairs.
III. Assumption of Risks
The Client acknowledges that they have been allowed to ask questions regarding the waxing procedures and have received satisfactory answers. By signing this Release, the Client assumes all risks associated with the waxing service, including those mentioned above, and agrees to proceed with the service voluntarily.
IV. Waiver of Liability
In consideration of being allowed to undergo the waxing service, the Client hereby waives, releases, and discharges the Service Provider, its officers, employees, and agents from any liability, claims, demands, actions, or causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by the Client during or as a result of the waxing service.
V. Indemnification
The Client agrees to indemnify and hold harmless the Service Provider, its officers, employees, and agents from any claims, demands, liabilities, losses, damages, expenses, and costs (including reasonable attorney fees) arising out of or related to the Client's participation in the waxing service.
VI. Consent
The Client hereby consents to the waxing service and acknowledges that they have read and understood the terms of this Release of Liability. The Client affirms that they are of legal age and competent to sign this Release.
VII. Agreement to Follow Instructions
The Client agrees to follow all instructions given by the Service Provider before, during, and after the waxing service to minimize the risk of injury or adverse reactions.
