Waxing Client Release Of Liability

This Release of Liability ("Release") is made effective as of [Date] between [Your Company Name], located at [Your Company Address], hereinafter referred to as the "Service Provider," and the undersigned client, hereinafter referred to as the "Client."

I. Purpose of Waxing Service

II. Acknowledgment of Risks

III. Assumption of Risks

By signing this Release, the Client assumes all risks associated with the waxing service, including those mentioned above, and agrees to proceed with the service voluntarily.

IV. Waiver of Liability

In consideration of being allowed to undergo the waxing service, the Client hereby waives, releases, and discharges the Service Provider, its officers, employees, and agents from any liability, claims, demands, actions, or causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by the Client during or as a result of the waxing service.

V. Indemnification
