Minor Release And Waiver Of Liability

I. Introduction

In consideration for being permitted by [Youth Sports League] ("the Organization") to participate in youth sports activities, including but not limited to practices, games, tournaments, and related events (collectively, the "Activities"), conducted by the Organization, I, [Parent/Guardian Name], on behalf of myself and my minor child, [Minor's Name], hereby acknowledge, agree, and represent as follows:

II. Assumption of Risks

Assumption of Risks: I understand and acknowledge that participation in youth sports activities involves inherent risks of injury, including but not limited to the risk of serious bodily harm, paralysis, or death, as well as property damage. I understand that these risks cannot be eliminated regardless of the care taken to avoid injuries, and I voluntarily choose to assume all such risks, both known and unknown, arising from or in any way related to my child's participation in the Activities.

IV. Release and Waiver

V. Indemnification

Indemnification: I agree to indemnify, defend, and hold harmless the Released Parties from and against any claims, demands, actions, liabilities, damages, losses, expenses, or judgments, including but not limited to attorney's fees, arising out of or in connection with my child's participation in the Activities, including any claims brought by third parties arising from my child's actions or conduct during the Activities.

VI. Medical Treatment

Medical Treatment: In the event of any injury or illness suffered by my child during the Activities, I authorize the Organization and its representatives to obtain medical treatment for my child as deemed necessary, including but not limited to emergency medical care, and I agree to be financially responsible for any costs associated with such treatment.

VII. Binding Effect

Binding Effect: This Release and Waiver of Liability shall be binding upon me, my child, and our respective heirs, executors, administrators, legal representatives, successors, and assigns, and shall inure to the benefit of the Released Parties and their respective successors and assigns.

VIII. Governing Law

Governing Law: This Release and Waiver of Liability shall be governed by and construed following the laws of [State/Country], without giving effect to any principles of conflicts of law.
