Affidavit of Separation

Statement of Separation
My spouse and I have separated as of January 1, 2051. We have not cohabitated or carried out marital duties since this date. Our separation is intended to be permanent and marks the end of our marital union. This separation has occurred under mutually agreed terms and conditions.
Reason for Separation
