Dear Michael Smith,

I hope my message finds you well. My name is Jane Smith and I'm writing out of utmost concern for one's well-being. A situation has arisen that needs both of our attention and understanding.

Unfortunately, I have been experiencing severe eye strain for a while now. It has been impacting my capabilities to function optimally and efficiently. This eye strain has now reached a point where it requires immediate medical attention and a period of rest and recovery. In light of this, my doctor has strictly advised me not to engage in any forms of exhaustive work or learning process which involves significantly heavy usage of eyes.

Understanding the predicament this may cause for my assigned tasks or obligations, I am fully committed to cover for the lost time. Upon recovery, I am more than willing to work double shift or take extra classes to catch up. Of course, it will be under the boundary of what could be deemed appropriate and manageable by the doctor. I hope you understand the situation I am in and consider the request for time off with a lenient heart.

The duration of my leave will depend on the doctor's recommendation but I assure you of my utmost diligence to resume my duties at the earliest upon recovery. It is never my intention to fall behind my schedule or duties, but prioritizing health at the moment seems the most sensible thing to do.
