Volunteer Waiver
I. VOLUNTEER WAIVER AND RELEASE OF LIABILITY
This Volunteer Waiver and Release of Liability (the "Agreement") is executed on this March 22, 2060 by the undersigned volunteer (the "Volunteer") in favor of [YOUR COMPANY NAME], its directors, officers, employees, agents, and volunteers (collectively referred to as "Organization").
II. VOLUNTEER ACKNOWLEDGEMENT AND ASSUMPTION OF RISK
The Volunteer understands and acknowledges that their participation in volunteer activities involves certain risks, including but not limited to physical injury, personal property damage, and/or emotional distress, which may result from activities related to the volunteer work.
The Volunteer agrees to assume full responsibility for any risks associated with volunteering and acknowledges that participation is voluntary.
III. RELEASE OF LIABILITY
The Volunteer, on behalf of themselves, their heirs, executors, administrators, and assigns, hereby fully and unconditionally releases and holds harmless [YOUR COMPANY NAME], its employees, agents, officers, directors, volunteers, and affiliates, from any and all claims, causes of action, demands, damages, and liabilities, whether known or unknown, arising out of or related to the volunteer work.
This release of liability applies to any injuries, accidents, or incidents that may occur during the volunteer activities or as a result of the volunteer's participation.
IV. MEDICAL TREATMENT
In the event of an injury or medical emergency, the Volunteer grants permission to [YOUR COMPANY NAME] to seek medical treatment on their behalf, and understands that they are responsible for all costs associated with such medical treatment.
V. CONFIDENTIALITY
The Volunteer agrees to keep any and all confidential information they may have access to during their volunteer activities confidential and not disclose it to any third party without prior written consent from [YOUR COMPANY NAME].
VI. USE OF NAME AND LIKENESS
The Volunteer grants [YOUR COMPANY NAME] permission to use their name, photograph, and likeness in promotional materials, including but not limited to websites, brochures, social media, and advertisements, without compensation or further consent.
VII. VOLUNTEER CODE OF CONDUCT
The Volunteer agrees to comply with all [YOUR COMPANY NAME] policies, rules, and guidelines while engaging in volunteer activities.
The Volunteer understands that failure to comply with these policies may result in their removal from the volunteer program.
VIII. ACKNOWLEDGEMENT OF UNDERSTANDING
By signing this Agreement, the Volunteer confirms that they have read and understood the terms and conditions set forth in this Waiver and Release of Liability. The Volunteer acknowledges that they are signing this Agreement voluntarily and with full knowledge of the potential risks and consequences.
