1. Acknowledgment of Risks
I, the undersigned employee, acknowledge that participation in company-sponsored events, including but not limited to team-building activities, retreats, and sports events, involves inherent risks. These risks may include, but are not limited to:
Physical injury (e.g., falls, sprains)
Emotional distress (e.g., stress, anxiety)
Property damage (e.g., loss of personal belongings)
Accidents or incidents (e.g., vehicle accidents)
2. Waiver and Release of Liability
By signing this waiver, I agree to release, indemnify, and hold harmless [YOUR COMPANY NAME], its employees, agents, officers, and representatives from any and all claims, liabilities, damages, or expenses, including legal fees, arising out of or in connection with my participation in the above-mentioned event, even if caused by the negligence of the Company.
3. Assumption of Responsibility
I understand and agree that:
I am voluntarily participating in this event.
I have the responsibility to adhere to all safety guidelines and instructions provided by the Company or its representatives.
I am responsible for my own actions during the event and will conduct myself in a safe and respectful manner.
4. Medical Treatment
In the event of an injury or medical emergency, I grant permission to [YOUR COMPANY NAME] to secure any necessary medical treatment for me. I understand that I will be responsible for any medical expenses incurred as a result of such treatment.
5. Severability
If any provision of this waiver is found to be unenforceable or invalid, the remaining provisions shall remain in full effect.

This waiver shall be governed by and construed in accordance with the laws of the state of Arizona.
7. Acknowledgment of Understanding
I have read this Employee Indemnity Waiver in its entirety. I understand its contents and agree to its terms. I acknowledge that I am signing this waiver voluntarily and with full knowledge of its implications.
