Free Simple Child Care Liability Waiver

Section 1: Acknowledgment of Risks

I, [Your Name], hereby acknowledge that I am the parent or legal guardian of Porter Hoppe. I understand that participation in childcare activities involves inherent risks, including but not limited to accidents, injuries, or illnesses that may occur while my child is under the care of Little Explorers Child Care Center.

Section 2: Assumption of Risks

I voluntarily assume all risks associated with my child's participation in the activities provided by Little Explorers Child Care Center, including but not limited to:

• Outdoor play

• Creative arts

• Organized games

I acknowledge that these activities may involve physical exertion and could lead to minor injuries such as bumps, bruises, or cuts.

Section 3: Release of Liability

I hereby release, waive, discharge, and hold harmless Little Explorers Child Care Center, its owners, employees, and agents from any claims, liabilities, or damages arising out of or in connection with my child's participation in these activities. This includes but is not limited to, any claims arising from the negligence of Little Explorers Child Care Center or any of its staff members.

Section 4: Legal Validity

I understand that this waiver is intended to be as broad and inclusive as permitted by the laws of the state of Arizona and that if any portion is held invalid, the remainder of the waiver will continue in full force and effect.

Section 5: Agreement

I have read and understood this waiver, and I agree to its terms.
