This Release of Liability (the "Agreement") made and entered into this day of, [DATE], by and between [YOUR NAME] hereinafter referred to as "Releasor", and [PARTICIPANT'S NAME], hereinafter referred to as the "Participant". I. Acknowledgement of Risks and Dangers
The participant hereby acknowledges and understands that by participating in organized basketball leagues or tournaments, they are potentially exposing themselves to certain risks, dangers, and hazards. These may not be limited and may include an assortment of specific situations. For instance, they could include injuries resulting from activities that are specific to basketball or the use of equipment associated with the sport. Furthermore, the actions and behavior of other participants in the league or tournament may pose an additional risk. The negligence of staff members, who may not handle situations correctly or make potentially harmful decisions, may also result in hazards. Moreover, a variety of meteorological conditions that unfold during the leagues could also pose certain risks.
II. Release and Waiver of Claims
The Participant, by executing this document, is herby agreeing to unconditionally release [YOUR COMPANY NAME], along with all its officers, directors, hired staff, representatives, and anyone working under its authority, umbrella, or guidance (collectively referred to as "Releases") from every potential or actual liability, lawsuit or legal action, claim, demand or cause of action. This applies to any of these that may potentially be law-based or equity-based, whether they are already known or unknown to the Participant at this time. This broad release includes anything arising from, associated with, or in any way connected to the Participant’s involvement in the basketball leagues or tournaments that are being organized and facilitated by [YOUR COMPANY NAME].
III. Acceptance of Terms
By participating in the organized basketball leagues or tournaments, the Participant acknowledges that he/she has read this release of liability, understands it, and voluntarily agrees to its terms. The Participant signs this release of liability willingly and voluntarily and will not make a claim against the Releasor for any damages or injuries sustained during the participation.
