Job Title: Care Coordinator
We, at Compassionate Care Connectors, are scouting for a tireless, empathetic, and organized individual to fill in as a Care Coordinator. In this role, you will be a critical part of our healthcare team, ensuring smooth coordination of patient care activities across our various departments.
Responsibilities and Duties:
You will find yourself assuming a wide array of roles—manager, liaison, advocate, and educator. On any given day, you could be orchestrating patient care activities across departments, scheduling and confirming patient appointments and follow-ups, or engaging in crucial dialogue between patients, their families, and the healthcare team to promote understanding of treatment plans.
Collaboration is key in this position. You will work with physicians, nurses, and other professionals in developing individualized care plans for patients, participating in patient progress discussions and any necessary refinements to their care paths.
You will educate patients and families about their health conditions, treatments, and processes, and be a sturdy advocate for their needs and preferences with their healthcare providers.
Qualifications, Skills, and Competencies:
Candidates for our Care Coordinator position should possess a relevant degree in healthcare or related fields and have professional experience in a similar role. Excellent administrative, communication, and organizational skills are a must. A high degree of empathy and a patient-centric approach is crucial. Knowledge of healthcare regulations, medical terminology, and ability to work with varied healthcare providers is expected.
