Job Title: Discharge Coordinator
Heatherfield Health is seeking a dedicated and compassionate Discharge Coordinator to join our team. The Discharge Coordinator will work closely with our patients to ensure a seamless transition from the hospital or other healthcare facilities to their homes or alternative care settings most efficiently and beneficially.
Qualifications and Skills
The candidate should possess a bachelor's degree in Nursing or Social work and have a valid practitioner license in the state of Hawaii. Excellent communication, time management, and organizational skills are needed in this position. Strong decision-making abilities and the capability to empathize with patients are also essential.
Responsibilities and Duties
As a Discharge Coordinator, your duties will include coordinating and planning the safe discharge of patients and providing families with essential information about post-discharge care, medications, and necessary follow-up appointments. You will facilitate patient-centered and high-quality care before and following discharge from hospital or healthcare facilities.
Experience
Applicants should have a minimum of two years of experience in similar roles in a hospital setting. Exposure to patients with various medical conditions is preferred.
Additional Requirements
Proficiency in healthcare IT platforms and Microsoft applications is beneficial. Candidates must abide by patient confidentiality policies and HIPAA guidelines. Willingness to work flexible hours including evenings and weekends is necessary to accommodate patient schedules.
