Job Title: Medical Case Manager
We are looking for individuals dedicated to ensuring patients receive appropriate and comprehensive healthcare services. Medical Case Managers are responsible for managing patient care across multiple settings, serving as a liaison between patients, families, and healthcare providers, and providing guidance towards effective treatment and healthcare plans.
Responsibilities and Duties
• Ensure appropriate care plans are established and followed.
• Collaborate with a multidisciplinary team to manage patient care.
• Advocate for patient’s needs and coordinate with healthcare providers to meet those needs.
• Monitor the effectiveness of the care plan and make adjustments as necessary.
• Maintain comprehensive case management records.
Qualifications
• Bachelor's degree in healthcare or related field.
• Must be a registered nurse (RN).
• Certification in case management (CCM) preferred.
• Experience dealing with chronic illnesses, disabilities, or complex conditions essential.
Skills and Competencies
• Strong understanding of healthcare management.
• Excellent interpersonal communication skills.
• Ability to advocate for patients.
• Proficiency in handling emergency situations.
• Able to identify resources effectively.
Necessary Experience
• Minimum of three years’ work experience as a case manager or in a similar role within a healthcare setting.
• Proven experience in developing healthcare plans.
