Job Title: Community Nurse
Evergreen Elderly Care seeks a dedicated and compassionate Community Nurse to provide top-quality care to patients within their residences. This position plays a crucial role in maintaining the health and overall well-being of each patient. The ideal candidate will have a comprehensive understanding of health assessments, care planning and implementation, and patient education.
Responsibilities and Duties:
Perform comprehensive health assessments to identify patient needs, monitor vital signs, symptoms, and overall health status.
Evaluate and address various factors affecting patient health, including psychosocial, cultural, and environmental aspects.
Devise and execute care plans aimed at health maintenance, disease prevention, and recovery.
Administer medications and treatments—as prescribed by healthcare providers—and perform nursing interventions as needed, including wound care and injections.
Assist patients with everyday life activities who need support.
Educate patients and families about health conditions, treatment alternatives, preventive measures, and promote healthy lifestyles.
Guide on nutrition, exercise, and medication management.
Share information on available community resources and support services.
Qualifications and Competencies:
Registered Nurse (RN) License, exceptional interpersonal skills, attention to detail, and a high degree of commitment and flexibility.
Necessary Experience:
At least two years of nursing experience in a community or home health setting. Experience working with the elderly population is preferred. Proficiency in medical software is an asset.
Additional Requirements:
Must have a valid driver's license and reliable transportation. Must also be able to cope with emotional stress and be physically able to assist patients.
