Job Title: Discharge Nurse
SilverHaven Medical Center is seeking a qualified and dedicated Discharge Nurse. The ideal candidate will be responsible for evaluating patients' readiness for discharge and developing comprehensive discharge plans in collaboration with our healthcare team.
Position Summary:
The Discharge Nurse plays a crucial role in assessing patients’ physical, emotional, and psychological conditions before discharge. This includes understanding the patients' medical conditions, treatment plans, and medication regimens. You will work alongside a healthcare team to ensure every patient understands the post-discharge care.
Responsibilities and Duties:
Evaluate readiness of patients for discharge.
Create comprehensive discharge plans in collaboration with other healthcare professionals.
Provide clear and concise post-discharge instructions and address any concerns patients may have.
Qualifications and Skills:
A registered nurse with a valid license.
Excellent communication and interpersonal skills.
Familiarity with medical software programs.

Experience:
Minimum of one year experience as a Discharge Nurse in a reputable healthcare setting.
