Job Title: Home Healthcare Nurse
We are ComfortCare at Home, a leading care provider, looking for compassionate, dedicated professionals to join our family as a Home Healthcare Nurse. This role requires premium hands-on medical care, monitoring and support to ensure the wellbeing of our patients within the comfort and privacy of their homes. Responsibilities and Duties:

Perform initial assessments regarding patients' health conditions and medical histories.

Create personalized care plans in collaboration with patients, their families, and the healthcare team.

Monitor patients' vital signs, symptoms, and overall health during home visits.

Administer medications, treatments, and therapies as prescribed by physicians.

Perform nursing procedures like wound care, injections, catheter care, as required.

Educate patients and families on disease management, medications, and preventive measures.

Assist patients with activities of daily living (ADLs), including personal care and mobility.
Qualifications and Skills:

Active Registered Nursing (RN) license in Kentucky.

Minimum of one-year nursing experience, preferable in a home health setting.

Effective communication and relationship building skills, with an empathetic approach.

Proficient knowledge in nursing procedures and direct patientcare.

Exceptional organizational skills and the ability to handle multiple priorities.

BLS/CPR certification is preferred.
