Medical Release of Liability
This Medical Release of Liability ("Agreement") is entered into by and between the Clint Renner ("Participant") and [YOUR COMPANY NAME] ("Provider"). By signing this Agreement, the Participant acknowledges and agrees to the following terms and conditions:
