Release of Liability for Play at a Person’s Home
This Release of Liability (the "Release") is executed on this 1st day of January 2060 by Kitty Johns (the "Participant") in favor of [YOUR NAME] (the "Homeowner"). The Participant desires to engage in play activities at the Homeowner's home located at Sacramento, CA 94203. By signing this Release, the Participant agrees to the following terms and conditions:
1. Acknowledgment of Risk
The Participant acknowledges that play activities at the Homeowner’s home may involve inherent risks, including but not limited to physical injury, property damage, or other harm. The Participant agrees to assume all risks, whether known or unknown, and accepts full responsibility for any damages or injuries that may result from participation.
2. Waiver and Release

4. Medical Treatment
The Participant authorizes the Homeowner to provide or seek medical treatment for the Participant in the event of an injury or other medical emergency. The Participant releases the Homeowner from any liability for providing or failing to provide medical treatment.
5. Representation
The Participant represents that they are in good health, have no physical conditions that would prevent safe participation in play activities, and have adequately prepared for participation.
6. Governing Law
This Release shall be governed by and construed in accordance with the laws of the State of California. Any legal actions or claims arising out of this Release shall be brought in the courts located in Sacramento, California.
7. Severability
If any provision of this Release is found to be unenforceable or invalid, the remaining provisions shall remain in full force and effect.
