8 Steps to Make an Incident Statement Form

Step 1: Decide the Structure

The first step in writing down an incident statement is to decide on the structure you are going to follow. Usually, different institutions have different structures provided to the officials. If this is not the case, there is a simple and common structure that you can follow mentioning the personal details of the people involved or the witnesses or victims, then providing the detailed description of the incident that has occurred.

Step 2: Personal Details

Once you have selected or are aware of the structure to follow in the making of the log statement, you need to provide sections for the personal details of the people involved or the witnesses or victims. These sections will include the name, age, sex and address of the concerned people. A separate section for the contact details of the people also needs to be provided. In some cases, people wish to remain anonymous. In that case, this portion is to be left blank.

Step 3: Provide a Summary

The next step after the personal details is to provide a section for writing an overview or summary of the incident. When the required person starts writing the incident statement, he or she needs to start with an overview of the incident. This means you need to describe the incident. This overview does not need to be detailed. Just providing a sentence about the occurrence will also do. You have to keep it as brief as possible.

Step 4: Incident Details

This is perhaps the most important step of the incident statement form. Sections and sub-sections if needed, need to be provided in the form that will be used to contain information about the incident that has occurred. This includes the basic facts and details of the incident such as the date, location and time of the incident, names of the people involved, etc. Separate columns should be provided for each heading.

Step 5: Provide Accurate and Correct Information

Make sure that the information you are providing is correct and accurate. If the information is not known by you fully, then do not include that piece of information. Be clear in what you are writing and be honest. Don’t use jargon or flowery words to describe the incident.

Step 6: Provide Factual Details

An incident statement form must contain only factual details. It should be meaningful and factually endorsed. Avoid including in the document any kind of emotional, judgemental and partial claims. There should be allocated space in the form for the person in charge to write about both sides of the story.

Step 7: Include Graphics

Graphical matters such as photos, illustrations, drawings, videos, and audio files make up for the part of supporting evidence. It complements the mentioned statistics and offers further information that the receiver can readily understand. That is why you need to provide ample space in the form to include such graphics.

Step 8: Proofread, Validate and Submit

Finally, when you decided upon and selected what elements to include in your form, you can make a rough draft. Since this is an official document, be sure to proofread it for any grammatical or spelling errors. If you are satisfied with the rough draft, get it printed. Some institutions’ protocols require the forms to be validated by the concerned authorities so make sure to do that if that is the protocol of your institution. Once all these formalities are completed, you can fill up the form with the required information and submit it.
