This Author’s Release Form ("Agreement") is made effective as of [Date], by and between [Author Name], located at [Author Address] ("Author"), and [Publisher Name], located at [Publisher Address] ("Publisher").
Grant of Rights
The Author hereby grants the Publisher the exclusive, worldwide, and irrevocable right to use, publish, and distribute the work titled "[Title of Work]" ("Work") in all forms, formats, and media now known or hereafter devised.
Copyright
The Author affirms that they are the sole creator of the Work and holds the copyright to the Work. The Author agrees to transfer all rights to the Publisher, subject to any limitations set forth in this Agreement.
Compensation
The Author will receive [Details of Compensation] for the use of the Work, payable upon execution of this Agreement or [other agreed terms].
Warranty
