Authorization: By signing this form, the customer authorizes the repair shop to perform the repairs and services listed above on the vehicle described.
Liability: The customer acknowledges that the repair shop is not liable for any damage to the vehicle unless directly caused by negligence during the repair process.
Payment: The customer agrees to pay for all services rendered as outlined in the repair estimate. Final payment is due upon completion of the repair.
Release of Liability: The customer agrees to release the repair shop from any future claims related to the services provided, except those caused by intentional misconduct or gross negligence.
Unforeseen Costs: If additional repairs are required once the vehicle is disassembled or during the repair process, the repair shop will contact the customer to approve any additional costs.
Storage Fees: If the vehicle is not picked up within 3 days after repairs are completed, storage fees of $ per day will be applied.
