Accident Description
Liam Parker was playing on the school’s playground during morning recess when he lost his balance while climbing on the jungle gym. As a result, he fell approximately 5 feet to the ground, landing on his left arm. The impact caused him to cry out in pain, and several students immediately alerted the playground supervisor, Ms. Emily Brooks.

Immediate Action Taken
Ms. Emily Brooks provided first aid and applied a cold compress to Liam's arm. The school nurse, Mrs. Linda Hughes, was immediately notified. Liam was escorted to the nurse’s office for further evaluation. After assessing the injury, the nurse recommended that Liam be sent home for further medical attention.
Outcome
