Recitals

WHEREAS, the Facility and the Resident entered into a Nursing Home Legal Contract ("Original Contract") dated [Month, Day, Year], under which the Facility has agreed to provide certain living accommodations and care services to the Resident;

WHEREAS, the Facility and the Resident desire to amend the Original Contract to reflect changes in services, fees, or other provisions as detailed herein;

NOW, THEREFORE, in consideration of the mutual promises contained herein and intending to be legally bound, the parties agree to amend the Original Contract as follows:

1. Amendments to Services

1.1 Changes to Provided Services: The services provided under the Original Contract are hereby amended as follows:

1.2 Adjustments to Service Levels: The level or extent of certain services provided under the Original Contract is hereby adjusted as follows:

  • Increased Housekeeping Frequency: For residents in private rooms, housekeeping services will increase from bi-weekly to weekly to ensure a higher standard of cleanliness.

  • Pharmacy Services Expansion: The on-site pharmacy will extend its hours of operation and offer a broader selection of over-the-counter medications and health products.

2. Amendments to Fees and Payment Terms

2.1 Adjustment of Fees: The fee structure outlined in the Original Contract is hereby amended as follows:

  • Room and Board Rate Increase: Due to rising operational costs, the monthly rate for room and board will increase by 5%. This adjustment reflects enhancements in resident care services and facility maintenance.

4. Amendments to Facility's Rights and Obligations

5. General Provisions
