DISCIPLINARY LETTER SAMPLE
Dear Tony Stark,
On January 24, 2020, you attended a disciplinary hearing because of your misconduct and misbehavior. I am writing to inform you that after careful deliberation of all the facts presented and hearing all sides, it has been decided by management to dismiss you.
Your last day of reporting to work will be on January 31, 2020. You will be given your final payment based on your last pay and accumulated pro-rated holiday pay.
