Liability Release
I, the undersigned owner of the above-mentioned dog, agree to the following terms and conditions:
Health and Safety: I certify that my dog is in good health and up to date on vaccinations. I acknowledge that grooming can sometimes expose hidden medical conditions or exacerbate existing ones.
Aggressive Behavior: I understand that if my dog behaves aggressively or bites during the grooming session, the groomer may discontinue services for the safety of my pet and the staff.
Matting and Shaving: I acknowledge that if my dog’s coat is matted, the groomer may need to shave the affected areas, which may cause skin irritation. I accept that this may be necessary for the comfort and health of my dog, and I will not hold the groomer liable for any effects of shaving.
Injury and Emergencies: While every effort is made to ensure the safety of all pets, accidents can happen. I understand that in the unlikely event of injury or emergency, I will be contacted immediately.
Payment and Cancellation: I agree to pay in full for the grooming services at the time of pick-up. I understand that cancellations must be made 24 hours in advance, or I may be charged a cancellation fee.
By signing below, I acknowledge that I have read and understood the terms and conditions of this release form. I release the groomer and the grooming salon from any and all liability, damages, or claims related to the grooming of my dog.
