This letter serves as a written warning  as our quarterly evaluation reports have revealed an unsatisfactory performance on your part. Your performance was being monitored by the management of the company and it has been observed that the ratings were not satisfactory. 

Take a note that your performance would be stringently reviewed in the coming months, and a final decision would be taken depending on your performance in these months. 

I will schedule a meeting at the end of the upcoming evaluation period and we will discuss your improvement. If you continue to show unacceptable performance, your employment may be subject to termination. Please be guided accordingly.
