Dear Ms. Bible,

Over the last few months, we have observed a decline in your work quality and adherence to the company's policies. The purpose of this note is not to admonish but to delineate specific aspects that require immediate attention.

Missed deadlines, issues relating to the quality of work delivered, and a few policy violations have been observed in your performance. Performance is a criterion that can affect both individual growth and company's productivity. Hence, we advise corrective action at the earliest.

This letter serves as a formal notice, and failing to improve performance may lead to further disciplinary action, up to and including termination. Please acknowledge receipt and understanding of this letter. Should you have any questions or need to discuss this matter further, feel free to contact us.

Best Regards,

Ronald Constable
