We are specifically concerned with your lack of motivation to reach your daily targets. Over the past few weeks, you have failed to improve your performance despite the best efforts of others within your team. This serves as a written warning since on the same issue. The attached document provides details on the same.

We would like to make it clear to you that we take employee performance seriously and any improper behavior will be subject to critical observation or examination and when under extreme necessity, disciplinary actions shall be imposed. Thus, we shall highly appreciate it if you take this written warning letter as a reminder to improve your work performance.

You are officially on probation starting immediately and extending until January 25, 2020. We will monitor your progress starting then and hope to see improvements. After the end date, we will evaluate your performance.

Your negligence to follow the conditions of this written warning and continued unsatisfactory performance shall lead to more severe and grave disciplinary action up to and including the termination of your employment.
