The intent of this letter is to remind you of your outstanding balance. We are urging you to have this balance paid within the stipulated time period from the date of this notice or contact our office for further information. We have attached the balance statement for your reference.
 
We accept any kinds of card payments. Simply fill out the payment form attached to this letter, sign, and send the letter back to the office. We are open to financial arrangements should you consider other payment options.
 
If payment has already been sent, please disregard this letter and accept our apology for the inconvenience this may have caused you.
