PARENT WAIVER LETTER

To Whom It May Concern,
I, [Your Name], am the parent/guardian of Emily Smith, who is currently enrolled in the Summer Adventure Camp organized by Bright Future Academy.
I understand that participation in the Summer Adventure Camp involves various activities that may include physical exertion such as hiking, swimming, and team sports, as well as interaction with other participants and the use of equipment like climbing gear and water sports equipment. I acknowledge that these activities carry inherent risks, including but not limited to, injury, illness, or property damage.
I hereby waive and release Bright Future Academy, its employees, agents, and volunteers from any claims, demands, actions, or causes of action arising out of or related to any injury or damages that may occur during my child's participation in the Summer Adventure Camp.
I certify that my child is in good health and has no medical conditions that would interfere with her ability to participate safely in the Summer Adventure Camp. My child has been vaccinated against common illnesses and has no known allergies that would impact her participation. I agree to inform Bright Future Academy of any changes in my child’s health or medical condition that may affect their participation.
In the event of an emergency, I permit Bright Future Academy to seek medical treatment for my child. I agree to be responsible for any medical costs incurred as a result of such treatment.
By signing this waiver, I acknowledge that I have read and understood the terms and conditions outlined above and agree to be bound by them.
