Dear [RECIPIENT NAME],

As discussed to you in our previous meeting regarding your successive absences, it is with regret that we formally inform you that we are pushing through with your termination from the company.

Based on company policy, [NUMBER] consecutive absences without prior notice is a major offense, which after due procedure, shall result in employment termination. We have given you several warnings yet you still did not provide due notice to your successive absences. This type of behavior is not tolerated, as per the company’s policy.  The decision to terminate your employment was made by all the members of the board, hence all the relevant factors with regard to your termination has been reviewed and agreed as a whole.
