Responsibilities and Duties

Regularly monitor and evaluate the effectiveness of care plans, and make necessary adjustments to optimize clients' well-being.
Advocate for clients' rights and needs within the healthcare system and the community, ensuring they receive appropriate services and support.
Qualifications, Skills, and Competencies
A minimum of a bachelor's degree in social work, nursing, psychology, or a related field.
Proven experience as a case manager or in a similar role.
Excellent understanding and knowledge of case management principles and healthcare practices.
Strong interpersonal and communication skills to effectively collaborate with clients, families, and healthcare professionals.
Ability to advocate for clients and navigate healthcare services and resources successfully.
Superior organizational skills, with the ability to prioritize and manage multiple tasks.
