Job Title: Care Coordinator

The Care Coordinator will be responsible for conducting comprehensive patient needs assessments and coordinating with other healthcare professionals to develop personalized care plans.

Responsibilities and Duties:

As our Care Coordinator, your core duties include:

• Performing thorough patient assessments, including their medical, social, and emotional needs.

• Working collaboratively with patients, their families, and our healthcare team to create individualized care plans.

• Regularly reviewing and updating these plans, ensuring they appropriately cater to the changing conditions of patients.

• Assisting patients in accessing necessary healthcare services such as medical appointments, diagnostic tests, and therapeutic interventions.

• Coordinating logistics for patient services, including arranging transportation as required.

• Connecting patients with community resources and support services to address non-medical needs.

Qualifications, Skills, and Competencies:

The ideal candidate will have:

• A relevant degree and certification in a healthcare field.

• Proven experience in a similar role, ideally within a senior living or healthcare setting.

• Excellent organization, planning, and coordination skills.

• Strong communication and interpersonal skills, with the ability to engage effectively with families, patients, and healthcare providers.

• Proficiency in maintaining patient records and privacy in accordance with HIPAA regulations.
