Maternity & Paternity Leave Contract
This Maternity & Paternity Leave Contract ("Contract") is entered into on January 28, 2076, by and between [Company Name] ("Company") and Sheila Mayes ("Employee").
1. Maternity Leave

If unforeseen circumstances prevent this notice, the Employee shall notify the Company at the earliest possible opportunity.
