Free Sperm Donor Agreement

This agreement is entered into on July 10, 2050, between [Name of Sperm Donor], hereafter referred to as the "Donor," residing at [Donor's Address], and [Name(s) of Recipient(s)], hereafter referred to as the "Recipient(s)," residing at [Recipient's Address].

1. Assisted Reproduction Procedure

1.1 Assisted Reproduction Method: The party or parties, who are being referred to as the Recipient(s) in this particular context, along with the entity or individuals labeled as the Donor, are in mutual agreement that they will proceed with the medical procedure known as intrauterine insemination (IUI). The sole and primary purpose for the utilization of this technique is their shared intention to conceive a child.

1.2 Medical Facility or Clinic: The parties involved in this agreement fully acknowledge and understand that the medical procedure of assisted reproduction will be carried out and performed by the said medical facility or clinic, specifically known as Fertility Solutions Clinic.

2. Donor's Obligations

2.1 Medical Screening and Testing: The individual who is donating, herein referred to as the Donor, concurs and is in agreement to participate and go through the necessary medical examinations, screenings, and testing procedures. These procedures, which may be required and dictated by Fertility Solutions Clinic's terms, mandates, and guidelines, are done with a specific goal in mind: to carry out precise, accurate, and necessary verifications regarding the suitability, or the fittingness, of the donated sperm. This is particularly important in the context of using the said sperm for assisted reproductive techniques and procedures. Therefore, the Donor pledges to undergo these essential health checks to ensure safe and effective assisted reproduction.

2.2 Health Affirmation: The Donor hereby confirms and states affirmatively that his current health status is sound and good to the best of his knowledge and awareness. Further, he certifies that he is not suffering from any conditions relating to sexually transmitted infections. Additionally, he also affirms that, according to his belief and understanding, he does not have any genetic disorders.

2.3 Sperm Sample Provision: The individual who is donating, henceforth referred to as "The Donor", agrees and is obliged to deliver samples of his sperm at times and locations that have been discussed and decided upon in advance by the Fertility Solutions Clinic.

2.4 Legal and Financial Responsibility: The Donor agrees that he will not have any legal or financial responsibility for any child conceived through the assisted reproduction procedure.

3. Recipient's Obligations

3.1 Medical Procedures and Treatments: The Recipient(s) agree to undergo medical procedures and treatments as required by Fertility Solutions Clinic to facilitate the assisted reproduction process.

3.2 Financial Responsibility: The Recipient(s) agree to bear all costs associated with the assisted reproduction procedure, including but not limited to medical expenses, fertility treatments, and clinic fees.

3.3 Indemnification: The Recipient(s) agree to indemnify and hold harmless the Donor from any claims, liabilities, or expenses arising from the assisted reproduction procedure or the resulting child.

4. Parental Rights and Responsibilities

4.1 Legal Parenthood: The parties acknowledge that the child conceived through assisted reproduction will be the legal child of the Recipient(s), and the Donor will have no parental rights or responsibilities.

4.2 Assumption of Parental Rights and Responsibilities: The Recipient(s) agree to assume all parental rights and responsibilities for the child, including but not limited to custody, support, and decision-making authority.

5. Confidentiality

5.1 Agreement Confidentiality: It has been mutually decided and agreed upon by all parties involved that they will uphold an extremely high level of confidentiality about the specifics of this agreement. In addition, how the process of assisted reproduction is executed will also be held in strict confidence.

5.2 Information Disclosure: The parties involved in this agreement have reached a mutual understanding that they shall refrain from revealing any sort of information that could potentially identify each other. However, an exception to this rule will be made if and only if there is written consent provided beforehand specifically approving the disclosure of such identifying details. Another exception may arise if it becomes necessary for such information to be disclosed due to legal obligations or requirements necessitated by the law.

6. Termination of Agreement

6.1 Mutual Termination: This agreement can be terminated if all the parties involved mutually consent to the termination and express this agreement in a written format.

6.2 Obligations Upon Termination: If it so happens that the agreement is terminated, both parties involved have agreed to adhere to and complete any outstanding obligations that are stipulated under the terms of this agreement.

7. Governing Law

The understanding and interpretation of this agreement shall be controlled and directed in compliance with the laws that are mandated and established by the jurisdiction that shall be specified.

8. Entire Agreement

This agreement constitutes the entire understanding between the parties concerning the subject matter herein and supersedes all prior agreements and understandings, whether written or oral.
