How to Make/Create a Survey [Templates + Examples] 2023

A survey is a document designed to gather data regarding a particular subject. This document is distributed to a group of people to gather insight, knowledge, and opinion based on the questions provided. 

Build a Survey – Step-by-Step Instruction

Surveys are used in business and academics as a means to understand and comprehend a certain topic or subject matter. Information collected from surveys plays an integral role in any decision-making process.

Step 1: Look for a Survey Template (Option 1)

To help you create a survey, your best option is to look for a survey template in Template.net. You will find a wide selection of templates that come in a variety of formats and designs.

Step 2: Use the Search Tab (Option 2)

If ever you have a specific survey template in mind, for example, a house cleaning survey then you can use the search tab and type the keywords on the search text box. 

Step 3: Choose a Template You Can Work With

Choose a survey template that meets your requirements by simply clicking on the image. You will be then redirected to a page similar to the one shown above.

Step 4: Select a File Format

You need a program or application in order to edit the template. That said, select a file format from the drop-down menu located right beside the template image and click the ‘download now’ button.

Step 5: Open the File and Start Editing

After you have downloaded the file, you can now start editing the template.

Step 6: Add a Title

Begin with adding a title. This should be printed on the uppermost portion of the document. Use a slightly bigger font size and in bold letters so that participants can identify what the document is all about.

Step 7: Include an Introduction

It is important to include a brief introduction (around 1-3 sentences long) right after the title. This could be an explanation of what the survey is all about or an instruction.

Step 8: Write Down the Questions

The next section and the most important one are the questions you will be asking the participants. Now there are several ways to present questions, such as open or closed-ended questions, simple Yes or No, nominal questions, or Likert scale questions. Make sure that the questions are simple and easy to understand for your participants.  

Step 9: Contact Information

Do not forget to add any contact information to the survey. Participants may want to reach out in case they have any additional questions or concerns. 

Step 10: Save or Print the Survey

After you have finished preparing the survey, you have the option to save or print the file.
