Step 1: Open Google Forms
A Google account is needed to use and access Google Forms. Click on the Google apps icon located at the top-right section of your Google home screen just beside your profile photo.

Step 2: Click on Blank Form
Click on Blank form to get started, or choose from the wide array of premade templates at the Template Gallery.

Step 3: Name Your Form
Change the Untitled Form to the name of your form, and add a form description just below the title.

Step 4: Customize Form
Navigate through the paint palette icon to customize your form. Choose the header, form color, background color, and font style.

Step 5: Encode Questions
Start typing in your questions.

Step 6: Drop Down the Modes in Answering
The default mode in answering the questions is Multiple Choice, however, you have the option to choose among short answers, paragraphs, checkboxes, drop-down, file upload, linear scale, multiple choice grid, checkbox grid, date, and time.

Step 7: Choose File Upload
Click on File Upload if you allow your respondents to upload a file. Read the reminder and click “Continue” to proceed.

Step 8: Manage Details
Manage all necessary details in uploading a file, including the allowing of specific file types, the maximum number of files, and maximum file size which you can change by simply clicking on “Change”.

Step 9: Continue with Questions
Continue encoding your questions in the next question section, you may duplicate files, add files, add images or videos, cut sections, manage the “Required” switch, and so on.
